How to register at this practice
If you are resident within our designated practice area (please click for the map on our home page), you can register with the practice. You will be asked to complete an official NHS registration form for each person or family member registering. You will also be given a new patient medical questionnaire and an ethnicity form from the practice, which provides us with basic medical information for the period until your notes reach the practice. May we request that you bring you NHS medical card in at the first opportunity.
When registering, you will be asked to provide 2 proofs of identity, one of which should show your address, and one should show photographic identity.
Unless you have complex medical problems or take multiple medications, you will be asked to make a new patient appointment with the practice nurse rather than the doctor. If you prefer, you may make this initial appointment with the doctor. Should you fail to keep this initial registration appointment without letting us know, you will not be allowed to join the practice.
Patients are now registered with the practice rather than with an individual GP. Your medical card will have a doctor’s name attached, but this is purely for administrative purposes. You may indicate your preference to see or speak to a particular doctor, but the practice cannot always accommodate these requests as specific doctors may not always be available.
