To ensure your medication is prescribed safely you are required to have a review of your HRT every 12 months. As some types of HRT can increase your risk of breast cancer or blood clots, it is important for you to remain vigilant for signs of breast cancer by regularly self-checking and attending screening when invited. Your annual review can now be done by completing an online questionnaire. If you have not received an SMS with details on how to access the form, please contact reception. Fill in as much information as possible. Some of the information will already be included in your medical records but this is a good opportunity to update any changes.

You will be required to submit an up-to-date weight and blood pressure reading and we will be unable to provide a repeat prescription without this. If you do not have access to a BP machine, you can make an appointment with the health care assistant in the surgery.

Please complete and submit the form at least 14 days before your next prescription is due. After this review, you will be issued a 12 month supply if appropriate. If you feel you need to discuss your medication, there is an opportunity to let us know at the end of the form and an appointment will be arranged. Please allow 14 days for your prescription to be processed. It will be sent to your preferred pharmacy unless you arrange otherwise for it to be collected. If you have difficulty accessing or completing the form, you can still arrange an HRT review in practice.
